Volunteer Application for Teen Camp

We ask all volunteers to please bring their own tent to camp, if at all possible. If you require other accommodations, kindly explain in the "Notes" section of this form. Please select your answer from the drop-down menu.
Volunteers have the opportunity to participate in many of the scheduled retreat activities. If you feel inspired and have the means, we would greatly appreciate your additional support. Please select your answer from the drop-down menu. The amount selected will be added to the minimum $25 application fee. The regular adult camping fee for this retreat would be $340.

Have you worked as a volunteer for the monastery before and if so, what was your assignment?
For those with volunteer experience at Deer Park, is there a service family that you would be happy to lead?
Is there a monastic currently residing at Deer Park who knows you and can act as a reference? If you do not have a reference, please answer no.

Medical Information

For your safety and the safety of others, we kindly ask you to answer the following questions to the best of your ability.
Please describe any allergies you have. Kindly note that we are unable to accommodate special diets and we do not have a cooking space for guests. If you are on a special diet, you may wish to bring some vegetarian food that does not require preparation to supplement your meals.
Please describe any ongoing medical conditions you may have and any medications you take for your physical health.
If you have been diagnosed with a psychiatric or psychological condition, please explain and list any medications you take for mental health.

By checking the box above, I acknowledge that I am applying for a mindfulness retreat volunteer program. If accepted, I understand that I am expected to participate in the daily volunteer schedule and to practice according to the Five Mindfulness Trainings during my stay (please see our website for more info). I understand that I am expected to remain on the monastery grounds for the duration of my stay, unless I receive permission to leave and entry information to reenter. Further, I understand that, except in cases of emergency, I will not have regular access to monastery phones, computers, internet, or Wi-Fi, and that outside work, school, and family obligations must be set aside during my stay.
